Motivhealth Health Medical Plans
The Mohave College plan offers a provider network of doctors and other health care professionals who have agreed to accept lower amounts than their standard charges for members of the Mohave College plan. These lower amounts are negotiated and predetermined. That means when you see a network provider, your share of costs is based on a lower charge—so your costs are lower, too.
Network providers are conveniently located in both urban and rural areas. Lower costs and convenient doctors and clinics are important ways that Mohave College can support your efforts to stay well and have a healthy lifestyle—or to have simple access to care.
When you need medical services you have access to providers in the MotivHealth Network.
Just visit Provider Search at www.motivhealth.com. You can find doctors by name, specialty and location. You’ll also find maps, directions, language options and more.
*AD= After Deductible is met
*Embedded deductible means each family member has their own individual deductible, up to the family deductible
*Non-embedded deductible means one combined, total deductible that must be met by all family members combined

What you’ll find on the MotivHealth® member portal
Using the MotivHealth member portal, you have 24-hour access to a number of tools and resources that can help you manage your health benefits.
1.Go to MotivHealth.com
Click “Log In.” At the sign in screen, you’ll click on the link to create a new account.
2. Enter your information
Provide the requested information. You’ll then be asked to create a username and password. Once your account has been finalized you will be asked to re-enter your new username.
